Style: kyokushin--
Its formal name is Domawashi kaiten geri. Although it is often called just Domawashi geri or just Kaiten geri.
It comes in basically 3 version:
Mae domawashi kaiten geri, where you roll straight forward in basically a straight judo forward roll
Yoko domawashi kaiten geri where you roll forward while turning sideways.
Ura Domawashi kaiten geri, where you spin around and a roll away from the opponent.
Its a bit hard to explain in words, so here is a vid that goes through all 3 version. The video creator calls them forward rolling kick (=Mae domawashi kaiten geri), Falling kick (=yoko domawashi kaiten geri) and falling back heel kick (=ura domawashi kaiten geri).
Its not a great instructional, but it gives a good view of the kick variations.

Yes. Kickboxing.
check out Peter Graham vs Badr Hari I (k-1 Oceania 2006)
Badr was out for something like 5-6 minutes, and shattered his jaw in multiple places.
Jadamba Naratungalang hit Albert Kraus with it in K-1 Max a few years ago. Kraus was saved by the bell (round 1), but was visibly affected long into the next round. He still won in the end, though.
A few other times, but names escapes me.

Awe, everyone who doesn't train karate feels the same way!
I'm actually impressed enough to facebook it to my instructor. We've been doing some spinning back drills and this is a good example. Note how he drops into a front stance (slo mo shows he lifts BOTH feet off the ground - he doesn't step back, but rather hops and slides both feet away from each other) at :07/:08 and is airborne in the actual kick is off the ground.
