Yes it can. Most injuries in swimmers occur in the shoulder/rotator cuff area. Many folks that swim for exercise swim in a way that puts them at risk for injuries to the region -- improper stroke technique, overuse of the freestyle/crawl, not breathing on both sides, etc. Adding weight increases the strain to an already vulnerable area.
