Interesting. I've never seen it broken down quite like that. I'm going to have to try some of those warm up techniques leading up to throwing the kick to see how it feels compared to what I do now. I don't really see the point in bringing both heels up under your ass, but maybe it's just a mental reinforcement thing. I was always taught the you only need to jump off the lead leg enough so that your foot is off the ground and not providing friction.
Edited to add: I think Beorn has teh correct on GSP's missed kick. One tell tale sign is he starts reaching with the ball of his foot when it should have become apparent that Hughes was out of range.
