Sometimes it takes a few seconds of struggling and gurgling, but sometimes if you hit it just right the guy goes out like a light. The best explanation I've seen is that you can activate the carotid sinus reflex causing a rapid blood pressure drop, which is much faster than the (very) partial constriction of the arteries in even a properly applied choke.
