I recently inherited a Donjoy Iceman. Basically, an igloo cooler with a water pump that keeps ice water circulating through a pad. I used it on my swollen knee last night (blocked a shin with my knee), wow, I do not know why I did not buy one of these before. Way easier than an ice pack, less messy too. Well worth the $50-60 I see them on ebay for.
Now I just need a hot tub for the opposite effect.

How does it work? You hook up an existing pad and then it pumps cold water in constantly?

Get the green Palmolive dishwashing liquid, put it in a zip lock baggy, then put it in another zip lock baggy, then freeze it. It's the cheapest and best ice pack you can find. My PT told me about this and I've had two frozen bags in the freezer ever since. They last about 30 minutes, are flexible, and really hold the cold. You can't use them against your skin without a dishtowel in between you and the bag. I used it for my shoulder surgery and a buddy is now using it for his knee surgery recovery.
