As one can probably see and surmise, I would prefer not to shoot the old hostage-taker-with-his-arm-around-a-hostage's-neck shot with my weak hand.

1. One hand injured. Weak-side-only is particularly big here, but if you really mean it, you practice one-handed reloading, too.
If you're the kind of guy who has to watch a violent movie to get the idea, see Way of the Gun.
2. Holding or holding back someone else--I go many places with two small children, not that I'm allowed to carry here anyway.
3. Applying pressure to a wound, either on you or someone else.
4. The aforementioned flashlight. As you mention, someone once decided that if you hold a flashlight against your jaw, as the FBI used to advocate, a bad guy who shoots at the light will probably get you. The proposed solution was to hold the light at arms' length. I've never made it work. -
