I think my understanding is that the DBs talk about training realistically with both sticks and empty hands as well as exploring which stick techniques can be effectively applied to empty hand combat. With sticks, you'll sparr realistically not only with a helmet but with hand and forearm guard - more than that would make it safer, but less realistic. When sparring hand-to-hand realistically, you would not use the same protective accessories as when sparring/fighting with sticks... my n00bish interpretation of what the DBs are talking about :5blindfol
