On the hand thing to clarify... there's a difference between hitting someone's head and someone's face right?
I've heard plenty of cases of breaking hands on people's domes, but frankly I find it unlikely that I'm going to punch someone in the chin or nose and have my fingers fall off.

Supreme excellence consists in breaking the enemy's resistance without spilling your Guinness.

Heh funny story: A good friend of mine broke his arm when he punched a guy in the hip accidentally during a drinking game (he was going for his drinking buddy's arm with full power and missed.)...
Also I have seriously bruised my thumbs when doing full resistance training with boxing gloves, just from having people block and jam my thumb, which is unpadded, with their forearm bone. The bruising seemed to go the core of both thumb joints and the pain lasted weeks (until I started repeatedly using jit-jat-jow formula on my thumb joints - that fixed it).
Humiliating story on a similar note: one time I was in a bad mood and very hung over and a fly landed on the window near me. It had been bothering me for a while so I slapped the fly, and put my whole forearm through the window. I gashed my wrist, but killed the fly. It was embarassing to explain that in the emergency room.

Then their punches suck. A good punch shouldn't break your hands if everything is lined up right.
Then again, I don't recommend hitting anyone without your gloves on. :) It might be assault otherwise.--
