Your best bet is to contact a health psychologist, as they specialize in a dualistic approach to mental health: Physical and psychological methods. Most psychologists focus on only the cognitive or subconcious aspects of treatment, and only give lip service to physical exercise and such.
"oh ya uh huh exercise beats depression and stress you should exercise"
One exercise that works well for me is to think about what is so stressful for me. Then I visualize myself confronting that stressful situation and confronting it successfully. Like, if I were nervous to go out on stage, I'd visualize the stage, I'd visualize myself walking to a podium, I'd visualize my speech, then visualize walking off. All confidently, all without worry.
A lot of what makes us nervous or unsure is unfamiliarity with unknown or stressful situations. If you repeat these situations over and over in your head, you've already done them that many times. -
