I agree with the headbutt+elbows. In brawls, I always try to strike with my palms, or elbows+knees. Less chance of a fucked up hand.

I'm not much for kicking, the only two kicks I can throw are a right roundhouse to the leg, or a front snap kick to the groin. But I've been working on throwing one after a good hand combo. my favorite so far is the left hook-straight right-right roundhouse to the leg.
I agree with what ojgsxr6 says about not just throwing combos but looking for openings. I will usually start off with a jab-straight right just to test their defense and see if they have any openings to exploit.

*From outside punching range, throw 2 seperate 1-2's at their guard with a lot of power. Make them fear those punches. The 3rd time, throw the jab normally, but FAKE the cross as you move into range. While they are covering, expecting that hard cross, pick your shot.
*Throw some back-handed snapping jabs at them, the jab should NOT be straight, it should be going outward. Make them expect an outside shot. When they start to parry the jab, charge in with 1-2-1-2 right down the pipe.
*Lead mid-kick to spinning backfist with rear hand. Catches a lot of people.
*fake jab to lead hook (1-3). Very effective when your opponent is reaching out to pat or parry your jabs.
*throw very hard 1-2's at opponents guard, keep doing it until they respect your power, and cover to defend. Next time you throw the 1-2, throw a wide and *visible* looping left (of any kind, the strike isn't important so much as them seeing the movement as a left hook). As you throw the left, quickly slip to the right. This should put you to the right side of your opponent with a clear shot at their temple and ear, while they are covering and expecting the left.

the one that always worked for me was a fast hard jab ro whatever arm they plan on blocking with(preferably on something hard) normally they'll move it (or it will move itself) a little and you can slide right past with a cross or hook.
I always figured **** if their arms are all fucked up they can't block..... or really punch for that matter
*I'm not saying punching someone's arm is going to mess it up to the point they can't use it, but it will more than likely move it out of the way a bit.
