Defence Unlimited (SBGi)
31 Spear Street
teaches BJJ, Stand-up and Vale Tudo classes
Organises a submission fighting league, has its own fight team. Cant find a fight record due to the patchy nature of the updates on the website, but his students do fight, and have won. Rosie Sexton being the most obvious example (I think shes 4-0-0)
For the rest of the UK, you'd be hard pressed to go wrong with one of the clubs listed here.

Inside a fitness center near Clark and Division in Chicago.
Carlson Gracie Sr. and Jr. teach here, along with several senior students with black belt proficiency in Carlson Gracie JJ. Carlson Sr does not roll with students or speak English, leaving it to other students to translate for him from Portuguese. As of my last visit, classes are usually crowded and well-attended.
