    I'd say when not doing so would put you in greater risk than doing so.

    Example, attacker has a knife and is about to cut you up. Your only way to get the knife out of play is to bite (say you can isolate the arm but not disarm properly). I'll take my chances with a strong series of heavy anti viral over getting turned into a BBQ prime rib piece.

    Why the **** would one headbutt with their head facing the attacker straight? As mentioned, you can break your nose, bust up the tissue over your eyes etc. I have been taught to turn your head a bit, tucking your chin in the direction of one shoulder and then use the side of your forehead because there is not much nerve tissue there, neither many blood vessels etc. to get you into trouble. You are also sorta save from upper cuts etc.
    And we trained that stuff on mits. The scenario was you either get control of their arms standing or you go to something like side mount/ mount/ half guard and then utilize it.

    I know you are not hitting with your nose (I understand the technique and I am aware of the use in CMA/JMA type of demonstrations) but for instance if the other guy just tucks his chin and explodes upwards he will smash your nose/teeth in and you will even help him.

    For the rest, I have no clue when it comes to the "medicine" stuff, while it makes sense to not hit with a twisted spine, I would assume the twist isn't that significant, is it?!? Wouldn't a hard hit, for instance during a boxing match, to someone who has his face turned, be even worse? And I am not aware of those type of injuries... It will make you more prone to KO because you can't absorb as well as with a straight spine but I am not sure if those two are the same thing.
