You want the one goodlun posted so. If you find it scary just make sure to squat as low as possible before you do the actual roll.

Good point, and I think the part where I really fail is kicking off with my legs. I don't commit 100% and that might create my failure. My Sensei says that he swears I am just about to go over and I either chest flop or land on my shoulder in a bad way. I don't know why I'm such a 'fraidy cat.

Your video shows the hands level, but with the fingers pointing in the direction of the toes i.e away from the head.

Your drawing, however, shows the hands level, but with the fingers pointing inwards towards the person's temples.

    Which one are you doing?

are you getting instruction during ukemi (breakfall) practice? this is the sort of thing that your instructor should be helping you with.

this is a decent video (although i hate to give people youtube examples for judo) that shows that breakfall (zenpo kaiten ukemi) from standing. i suspect as a beginner you may be doing it from kneeling however.

my gut feeling on this is that you aren't maintaining a slightly bent arm as you roll, and your arm is collapsing, resulting in you dropping all your weight on your shoulder, but again, without watching you do this, it's hard to tell.

you really need to ask your sensei for guidance on this.

"Without actual qualifications to be a Zen teacher, your instructor is just another roundeye raping Asian culture for a buck." - Errant108
"Seriously, who gives a **** what you or Errant think? You're Asian males, everyone just ignores you, unless you're in a krotty movie." - new2bjj

You don't actually need that much leg kick. If your head is tucked in nicely you pretty much just fall over and gravity takes care of the rest.

My Sensei has me do them with the hands pointed semi-inward.

Reading through the gymnastic post, it actually highlights a problem that I seem to be having:

If you're doing a gymnastics roll, then that's fine. However, a Judo zenpo kaiten ukemi/ mae mawari ukemi has the dominant hand placed in front palm downwards with the subordinate hand below it palm downwards.

My guess is you are mixing up the instruction you got for your break falls with the instructions you got for front rolls.

Yes. He only has me try 1-2 of these front-facing somersaults per practice. I have an easier time doing the breakfalls shown in the video you embedded, believe it or not. The first time I tried them, though, I did fall on my shoulder a few times so maybe the failed somersault just aggravated it.

I shall indeed, Ming Loyalist! I'm sitting here nursing the shoulder tonight so I figured I'd see if there was any input at all. :)
