As for your first question, the only thing that I can imagine is if they try and keep your guard and eyes high they're definitely going to shoot in low with something after. That could mean a good kick to the liver/ribs, or a sweep. That's really one big thing that I look for.
As for entries, my favorites would have to be the double jab to go for an inside leg sweep, or a good one-two punch (kizami-gyaku combo), and let your hip drive your back leg through the sweep. I've probably learned the most about karate sweeping from the gentleman in this video:
