• Do you still train in MA at least once per week?
Yes
• What type of gym/style do you train in currently?
I train stick fighting twice a week, tai chi every day, and some bag work thrown in there.
• How often do you engage in live sparring/rolling?
I spar sticks every class. Some if that is with padded sticks. We also roll and do empty hand sparring.
• How long have you been training in total?
Total of about 12 years, starting with kickboxing, LEO combatives, BJJ, Kung Fu, and now sticks.

Yes, I train 3-4 days a week currently, and 5-6 days a week when not working full time.
• What type of gym/style do you train in currently?
BJJ/10th Planet (and a bit of judo and wrestling thrown in...see what I did there?)
• How often do you engage in live sparring/rolling?
Every class.
• How long have you been training in total?
I did Karate for a year or two as a kid, five years of Aikido, and now about 3 years of BJJ.
• If you no longer train, what caused you to stop?
I stopped Karate because I was a kid and thought it was boring (in my defense, it was pretty crappy McDojo) and stopped training Aikido after I moved home and 1) didn't like the local dojo that much, and 2) burnt out on Aikido in a lot of ways and just didn't have a desire to keep training in it. -

Currently 3 times a week or more, since it's also my PT regimen

A handful of mats in a dingy gym building on a COP in Eastern Afghanistan!
• How often do you engage in live sparring/rolling?
That would require a partner who knows something about more than "Army Combatives"...who doesn't seem to be around here.
• How long have you been training in total?
In actual training, about 10 years. With gaps and stoppages caused by calls from my rich Uncle and lack of schools, about 17. Kenpo karate, about 1-1/2 years; hapkido, about 6 years off and on; and shotokan, about 2 years. Add in some crap "Army Combatives" to make sure I'm injured periodically and am training in NOT using effective techniques I already know... I prefer hapkido, but schools are hard to come by. Shotokan has a school near me, and is available on DVD and the internet. Not ideal but better than nothing, and I can work on Kata here in Asscrackistan.
• If you no longer train, what caused you to stop?
See above, but the most common reason for stopping is being sent to places that don't have dojo/dojangs. Lack of decent schools is second on the list.

When not injured yes. I train on average 3-4 days a week.
• What type of gym/style do you train in currently?
I train at an MMA gym. I'm currently training Judo and Jiu Jitsu (got my Blue Belt 2 weeks ago!), I've also done some of the boxing and MMA classes. I plan to join in the Muay Thai class as my schedule allows.
• How often do you engage in live sparring/rolling?
Every Jiu Jitsu class we roll so I roll usually 2x a week.
• How long have you been training in total?
11 years.
