Here are a couple of things that I am learning/working on a lot myself and noticing a lot in other people too.
many people can only strike when they are not moving and vice versa.
Keeping your feet underneath you, so that you don't come out of your stance and you are always ready to move, angle and punch/kick without overcomitting/leaning/reaching and coming out of your stance.
Using basic angling, distance and footwork so that you don't just run straight into people's power shots and die. -

I don't think this is advanced striking.
Anyway -
Lack of head movement, particularly after a strike
"dragging" hands (bringing them to shoulder/chest when retracting after a punch)
not changing levels for body shots
over-reaching/over-extending for the hook
bending over for the bob & weave
lack of variation in technique
chin coming up
turning away from opponent when you get hit
moving straight back instead of angling/circling off
head hunting - (more body shots please)

In a boxing context I see pretty much everything you mentioned, but this is definitely the biggest problem for strikers.
It's a bad habit, and it'll get you hurt. Seriously hurt.
..and the best bit is, the only cure is to get punched in the face moar!
